Important Changes to Dr. Spurney’s Practice in 2026


Dr. Spurney will be changing to a concierge practice on April 1, 2026. You may have received an email from her about this change.


As part of this transition, she will also be opting out of Medicare. This change affects the cost and billing of services provided directly by her. Specifically, as of April 1, Medicare will not provide reimbursement for any services she furnishes, and she will not submit

claims to Medicare on your behalf.


You may continue to use your Medicare coverage as usual for all other covered services and items, including prescriptions, specialists, diagnostic testing, procedures, hospital services, and other healthcare encounters.


Patients who have commercial insurance or Medicare can join her concierge practice based on the fees as described below.


Dr. Spurney will be limiting her practice to a smaller number of patients. This will allow her to spend more time with each patient and have time to answer messages, coordinate diagnostic testing and specialty care, and be able to offer timely appointments.


You may join a waitlist if her practice has reached capacity.


If you choose to enroll in her practice the options are:

  1. $2200 annual fee for one comprehensive physical appointment a year. Additional office or telehealth visits will be charged as fee for service based on the Internal Medicine Colleagues fee schedule. 

  2. $3300 annual fee for one comprehensive physical appointment a year, with additional total of three office or telehealth visits. Additional office or telehealth visits after the physical and 3 visits will be charged as fee for service based on the Internal Medicine Colleagues fee schedule. 


Dependents or patients 18-30 years old can stay in her practice as fee for service only, based on the Internal Medicine Colleagues fee schedule. 

Dr. Spurney will keep a limited group of patients on a fee for service basis only (no concierge fee) by special request.


WHAT ABOUT THE ADMINISTRATIVE FEE I PAID IN 2025/2026?

Any patient who chooses to enroll in this concierge practice who has already paid an annual administrative fee for 2025/2026 will have that fee deducted (in a pro-rated calculation) from this annual concierge fee for this 12-month period only.


Patients who choose to stay in the practice with another physician will not get a refund of their administrative fee.


Patients who leave the practice before 12 months has elapsed since they paid the fee can request a pro-rated refund (for example, if you paid the fee and you leave the practice 6 months later, we will refund you 50% of your administrative fee).


WHICH DOCTOR CAN I SEE IF I WANT TO STAY IN THE PRACTICE BUT NOT ENROLL IN DR. SPURNEY’S CONCIERGE PRACTICE?

If patients who currently see Dr. Spurney wish to consider staying in our practice, they can schedule an appointment with Dr. Chris Westley or Dr. Jeffrey Sherman. 


Dr. Spurney will continue to see her patients on a fee for service basis until mid- May 2026.


WHAT HAPPENS IF I WANT TO CHANGE TO ANOTHER DOCTOR OUTSIDE THE PRACTICE?

If patients choose to seek care with another provider, upon request, our office will facilitate the transfer of your medical records to any provider you designate, in compliance with applicable federal and state laws.

  • What are the fees for Dr. Spurney’s concierge practice?

    Dr. Spurney will offer two levels of concierge care plans. These are described below:


    Essential Plan

    $2200 annual fee for one comprehensive physical appointment a year. Additional office or telehealth visits will be charged as fee for service based on the Internal Medicine Colleagues fee schedule.


    Premium Plan

    $3300 annual fee for one comprehensive physical appointment a year, with additional total of three office or telehealth visits. Additional office or telehealth visits after the physical and 3 visits will be charged as fee for service based on the Internal Medicine Colleagues fee schedule.

    • Comprehensive lab testing appropriate for the annual physical

    • Telephone access to after hours physicians 7 days a week, 24 hours a day
    • Timely response to phone/email messages
    • Extensive review of outside medical records 
    • Coordination of care with specialists
  • What does the Premium plan include?

    • A comprehensive history and physical.

  • How are the fees charged for Dr. Spurney’s practice?

    Should you choose to enroll in the practice, the fee for the option you select will be payable by credit card or check on an annual basis. As long as you stay with the practice, you will renew this payment every 12 months. For example, if you choose to join in April 2026, you would renew your enrollment in April 2027.

  • Can my family members also see Dr. Spurney?

    Dr. Spurney will see patients who are 18-30 years of age and are dependents of patients in the practice on a fee for service basis.

  • Will Dr. Spurney practice at her current 2021 K Street, NW location?

    Yes. Dr. Spurney will continue to see patients at the current location 2021 K Street, NW, Suite 404, Washington, DC 20006. The phone number will remain the same, 202 466 8118.

  • How can I get in touch with Dr. Spurney?

    Dr. Spurney will continue to be available through phone calls to the office at 202 466 8118. Our office continues to have staff answer the phone in person rather than a phone tree. You can leave detailed messages with her office staff in a phone call. You can also reach her through the electronic record portal or email.


    Requests for appointment times, medication refills, routine orders for radiology or lab testing, and straight-forward clinical questions will still go through her office staff on the office phone or email. The staff will contact Dr. Spurney to get her input for these requests as needed. You may always ask Dr. Spurney to contact you to discuss questions about these requests.

  • What should I do if I am sick?

    You should call the office number at 202 466 8118. We will arrange for you to be seen in the office the same day or next day with Dr. Spurney, or Dr. Spurney will advise if you should seek emergency care.

  • What should I do if Dr. Spurney is out of the office or on vacation?

    There will continue to be 24/7 coverage through the office number 202 466 8118. This includes time sensitive prescription refills and urgent clinical situations.

  • Can I submit my fees to my insurance company for reimbursement?

    Please note that Dr. Spurney is still out of network for all commercial insurance plans. We will continue to provide you with paperwork for services you received at your appointment as usual. We may be able to help patients submit claims to their insurance on an individual basis. 


    Patients with Medicare plans will not be able to submit claims for services rendered by Dr. Spurney to Medicare. Please contact our office for more information about Medicare opt-out. We can provide you with a more detailed list of frequently asked questions about Dr. Spurney’s opted out status and what it means for you.

  • I have a Health Spending Account, Flexible Spending Plan, or a Medical Savings Plan through work. Can I use that to pay for the annual fee?

    You may be able to use these funds to pay for some or all of your annual fee. You should check with your plan administrator to see if you can use these funds.

  • What does Dr. Spurney's practice advise about lab testing?

    Dr. Spurney’s practice has an on-site phlebotomist who can draw and submit blood for laboratory testing through LabCorp. Patients can request a lab order from her office to take to an outside laboratory.

  • What should I do if I am in an Emergency Room or hospitalized?

    If you are in an Emergency room or hospitalized, you or a family member/friend should notify Dr. Spurney’s office if possible. This will allow Dr. Spurney to communicate with emergency room/hospital staff as much as possible to support your care. Hospitalized patients are primarily cared for by hospitalists (in-hospital doctors) and specialist doctors as needed.

  • How do I join Dr. Spurney’s new practice?

    You can call the office at 202 466 8118 and ask to speak with our medical assistant Nicole Mayhew or our office manager Rhonda Lindo to confirm your selected plan. We will send you a set of forms including a Physician-Patient Agreement and then you can submit payment for your selected annual plan fee via credit card or personal check.


    Dr. Spurney will accept up to a set number of patients. Once she reaches that capacity, patients will then be placed on a waiting list to be notified if a space opens in her practice.
